Before you come to your first appointment
You will need to download and complete registration and history forms. They help us get to know your unique story and needs.
Please, bring your photo ID and insurance card to your first appointment. If you unsure of coverage, call your insurance provider before our first session.
We try to keep our clinic running quietly and smoothly, with each clinician running their schedule independently. If you have any questions about the documents, feel free to ask your provider.
Arriving to your first appointment
We do not typically have a receptionist at our front desk. When you arrive you may be seated in the waiting room and your therapist will come to greet you at the time of your appointment. Your therapist will make a copy for you ID and insurance card and go through your completed paperwork with you. If you have any questions, feel free to ask your provider. We ask that you keep the waiting room respectful and calm, as it is a shared space.
